✅ Filled-up Application Form
✅ Voter’s Certification (Photocopy)
  (Must be Cebu City Registered)
✅ Medical Certificate signed by Doctor (Photocopy)
  (Within the month of application for chronic cases)
✅ Birth Certificate (if minor)
✅ Appearance / Submit Whole Body Picture
✅ Barangay Residency
