✅ Filled-up Application Form
✅ Photocopy of PWD ID Card with Number (Front & Back)
✅ Original Voter’s Certification (2013 below)
✅ Certificate of Disability
  (For Non-Apparent Disability)
✅ Original Copy of Medical Certificate signed by Doctor
  (Within the month of application)
✅ Certificate of Guardianship duly notarized
  (If both parents are deceased)
✅ Birth Certificate (if minor)
✅ Appearance / Submit Whole Body Picture
✅ Barangay Residency
  (For Financial Assistance Application)
